Informed Consent Cover Letter
STUDY TITLE
School Sports Concussion Surveillance Project
Dr. Richard Campbell, Ph.D., from the Department of Psychiatry and Behavioral Sciences, and Dr. Christopher McGrew, Ph.D., from the Department of Family and Community Medicine, as well as a multi-disciplinary group of professionals at the University of New Mexico Health Sciences Center, the New Mexico Athletic Association, and the New Mexico Brain Injury Advisory Council, are conducting a research study. The purposes of the study are to collect data to determine the incidence of youth sports concussions in schools in the state of New Mexico and how they are managed. You are being asked to participate in this study because you are involved in a school athletic program in the state of New Mexico and have been identified as the designated concussion reporter for your school.
Your participation will involve completing a concussion reporting tool that includes questions on suspected concussions sustained at your school. Specifically, you will be asked to report information regarding student-athletes who are suspected of having sustained a sports-related concussion, including age, gender, and grade/year in school, what sport was the student athlete participating in when the injury occurred, whether the student athlete was referred to a healthcare professional for evaluation, what type of healthcare professional was the student athlete referred to, whether a concussion was diagnosed, what healthcare professional cleared the student athlete to return to sports participation, what the length of time was from injury to return to play, and whether the student athlete has sustained prior concussions and if so how long since the last concussion. The survey should take no longer than 10 minutes to complete. Your involvement in the study is voluntary, and you may choose not to participate. You will be asked to provide the name of your school and your position. There will be an electronic link between your school and email address and your responses. Your responses will be housed within a study specific REDCap database, which is housed behind the Clinical & Translational Science Center of the University Health Sciences Center secure firewall protected environment. When survey procedures have been completed, data will be stripped of identifiers for purposes of analysis. No data will be analyzed or reported on an individual or school level. You can refuse to answer any of the questions at any time. The risk of breach of confidentiality is low and it is unlikely for individuals to experience discomfort when answering questions.
The findings from this project will provide information on number of youth sports concussions sustained in New Mexico and how concussions are managed. If published, results will be presented in summary form only.
